HealthSteps is a mobile health platform designed to connect people around the concept of a digital care plan. Through their own mobile devices, patients can be more fully engaged with their care plan activities and can share their digital care plan with family members and other caregivers providing a truly patient-centered solution leading to increased patient compliance and better health outcomes.
Operating on both iPhone and Android, the application leverages the smartphone's alerts and notifications to keep the patient on track with their care plan requirements. The patient's care plan can be loaded at the clinic site by scanning a QR code, or remotely by accepting a secure link sent by clinic personnel. The HealthSteps app then provides real-time reminders for care plan activities complete with detailed instructions, and additional links to multimedia resources like videos to enhance the patient's success with following their care instructions. HealthSteps allows symptom tracking and the capture of clinical data from wearables and bio-sensing devices, all in real-time and immediately viewable by clinic personnel.
Through the web portal clinic dashboard, providers can monitor progress of a patient's critical activities and receive alerts if key activities are missed. Providers can even adjust the digital care plan as needed upon review of patient-reported data or data from wearables or bio-sensing devices. Provider-to-patient messaging allows notifications to be sent directly to the patient for adjusting care plan activities, simultaneously allowing all connected family members and other caregivers to receive the same updates. By integrating HealthSteps with the electronic medical record (EMR), patient-reported outcome reports are received by the clinic, triaged by mid-level providers, and then forwarded through the existing EMR workflow to the appropriate provider.
At the heart of the HealthSteps app is the 'Share-n-Sync' feature, which connects a patient’s care plan with their network of family members, caregivers, and healthcare providers in a secure, HIPAA compliant environment. For pediatric patients, or patients with a surrogate caregiver, everyone is digitally kept on the same page.
